One vial contains 1 g of cefepime and L-arginine to maintain pH 4.0-6.0 of the solution. Cefepime is antibiotic of VI generation which can kill bacteria which are resistant to cephalosporin antibiotics of III generation. It is active against Staphylococcus, Streptococcus,Pseudomonas, Escherichia coli, Klebsiella,Enterobacter, Proteus, Aeromonas, Capnocytophaga, Citrobacter, Campylobacter, Gardnerella vaginalis, Haemophilus, Hafnia alvei, Legionella, Morganella, Moraxella, Neisseria gonorrhoeae, Neisseria meningitidis, etc.
This potent antibiotic is taken intramuscular or intravenously exactly according to prescriptions of your doctor. Solution for intramuscular injection is prepared on base of physiological solution or 1% solution of lidocaine. Fresh solution for injection can be stored not longer than 24 hours at room temperature or 7 days in refrigerator. To treat infections of urinary tract intramuscular injections of Cefepime 0,5-1 g should be made every 12 hours. Serious or life-threatening infections are treated with 2g of Cefepime intravenously every 12 or 8 hours depending on severity of infection. For children the dose should be corrected. Usual duration of treatment is 7-10 days. Severe infections require more prolonged treatment. Correction of dosage is required in patients with renal failure.
Before to administer this medicine, sensitivity of the patient to cefepime and identification of the pathogene should be determined via corresponding tests. Topra should be prescribed with caution to patients with any form of allergy. If allergic reaction develops treatment with Topra should be stopped. If diarrhea develops during treatment with cefepime than possibility of pseudomembranous colitis should be taken into consideration. Mild cases of colitis may pass on their own when treatment with Topra is stopped. Its moderate or severe cases may require special treatment. During treatment with Topra superinfection can develop, this requires discontinuation of treatment with the medicine and appropriate treatment. Safety of treatment in children under age of 2 months was not researched. The medicine does not affect the ability to drive or operate vehicles.
Do not take this medicine if you are hypersensitive to cefepime or L-arginine as well as to cephalosporin antibiotics, penicillin and other beta-lactam antibiotics.
Possible side effect
Topra is generally well tolerated. Side effects are rare. The most common side effects include diarrhea, nausea, vomiting, colitis, abdominal pain, allergic reactions such as anaphylactic reactions, rash, pruritus, urticaria; headache, dizziness.
Simultaneous administration of cefepime solution (and other beta- lactam antibiotics) with solutions of metronidazole, vancomycin, gentamicin, tobramycin and netilmicin sulfate a pharmaceutical interaction is possible. When cefepime is administered with the listed drugs, each antibiotic should be taken separately.
Take the missed dose as soon as possible. Skip the missed dose if it is time for your next scheduled dose. Don't take extra medicine to make up the missed dose.
Symptoms of encephalopathy are described for overdose of Topra. In such cases hemodialysis should be used especially in patients with renal failure. In pregnant and breastfeeding patients Topra should be used only under medical supervision. Cefepime is excreted into breast milk in very low concentrations. During lactation it should be used with caution.
Store the medicine at room temperature between 59-86 degrees F (15-30 degrees C) away from light and moisture. Store it in a tight container. Use only freshly prepared solutions for injections.
